serial cancellation worksheet

when a serial title is cancelled it affects multiple departments in our library: collection development; acquisitions; cataloging; and electronic resources.  in order to make sure each department gets the information it needs to perform a task related to a serial cancellation we’ve designed a worksheet to help.  a serial title cancellation is a process that doesn’t happen often enough that the discreet steps can be remembered easily, so having a worksheet with a series of prompts really helps.  i first saw a worksheet like this while i was employed at usc and have adapted it to suit our purposes at lmu.  i’ll link to the .pdf version here, so if it seems useful you can tweak it for your own institution’s use.

we’re still working on a worksheet for serial title withdrawals as well as title changes.  shoot me an email if you’d like to get a copy when we’re done.
